I hereby authorize Mr. Jophious Guiral to act on my behalf in all manners relating to
my PhilSat Requirements.

Last year, I have submitted all my requirements including my Original Copy of

Certificate of Eligibility for Civil Service. On this regard, I am respectfully asking for
your office to return the said document in behalf of my representative due to
distance constraint from Bacolod City to Manila, for it is one of the prerequisite to
Government employment which I am now handling.

Any acts carried out by Mr. Jophious Guiral on my behalf shall have the same effect
as acts of my own.

This authorization letter is valid until July 31, 2018.
