SUBJECT : REQUEST FOR ADDITIONAL FUNDS FOR THE COMPLETION OF CONSTRUCTION OF

TWO (2) UNITS – ONE STOREY WORKSHOP BUILDING AT TALIPAN NATIONAL HIGH
SCHOOL, PAGBILAO, QUEZON

Submitted for consideration is this letter of request for supplemental/buffer fund along with the Program
of Works for the Completion of Two (2) Units – One Storey Workshop Building at Talipan National
Highschool, Pagbilao, Quezon which needs an additional cost of P 786,494.05

The project allocation of P6,692,370.00 released under CY 2017 BEFF Batch 10 for National High School
(SARO No. BMB-A-17-0014250 dated September 7, 2017) was founded to be insufficient in completing
the project due to certain major modifications in the plans that are brought about by numerous involving
conditions which are to be promptly addressed. The said concerning modifications are as follows:

1) Due to disparity of the actual with the required land area of the initial plan, a compromise with
the design was adopted. A Change Order was made, converting the initial proposal of “Two (2)
Units – One (1) Storey Workshop Building” into a “Two (2) Storey – Four (4) Classroom School
Building”. Such revision of the design was endorsed by the school administration headed by its
school principal, Ms. Caridad C. Grimaldo.
2) supplementary materials are required for the additional structural members (e.g. Column, Beam,
Footing, etc.) involved in the modification. Such materials will be used for the augmentation of
the said structural components which are subjected to structural design changes in order to adapt
with the aforementioned plan adjustment.
