7:00-7:30 am Time of travel from CMU to BPH-Maramag/
7:30-7:45 am Preconference and checking of paraphernalia together with Practicing Clinical
Instructor
7:45-8:00 am Logging-in and endorsement
Listen to endorsements
8:00-8:30 am Entrances into the ward/ICU/Isolation/OPD-ER
Establish rapport with the patient and significant others
Assessment of the patient and Initial vital signs taking
Morning care and bedside care
Check available medications and IVs
8:30-9:00 am Drug Study
Preparation and administration of medications
Assist in the Doctor’s Rounds and carrying out of doctors’ order
9:00-10:00 am Continue morning care and bedside care
Construction of the DAR sample charting
Checking and plotting of vital signs for 10 am
10:00-11:30 am Continue morning care and bedside care
Drug Study, preparation and administration of medications
Construction of Final DAR sample charting
Carrying out of Doctor’s Order
11:30-12:00 nn Lunch Break – 1st batch (Endorse patient)
Preparations/Administrations of Medications for 12 nn and 1 pm
12:00-12:30 pm Lunch Break – 2nd batch (Endorse patient)
12:30-1:30 pm Construction and Checking of DAR sample charting
Preparations of medications for 2 pm
1:30-2:00 pm Transcription of DAR charts, checking and plotting of VS for 2pm
Review of the special orders/ endorsements during the shift
2:00-2:30 pm Close charting, checking and signing of charts by the clinical instructor.
2:30-4:00 pm Endorsement to Nurse-on-Duty
4:00 pm Exit from ward and Post conference
