LIMITED FACE TO FACE HOSPITAL DUTY
(SPECIFY ROTATION HERE)

GENERAL OBJECTIVE:

WEEK 1

At the end of 32 hours Limited Face to Face clinical duty as


a Student Nurse at the General Ward of Maria Rey – Xavier
University Hospital, I will be able to familiarize the physical setup
of the ward and the roles and responsibilities of the nurse, apply
the concepts of timely medical care and interventions,
demonstrate knowledge and skills in assessment and medical
management of illnesses, and deliver appropriate patient care
that is effective for the treatment of conditions.

SPECIFIC OBJECTIVES:

DAY 1 & 2

At the end of 8 hours Limited Face to Face clinical duty as a


Student Nurse at the General Ward of Maria Rey – Xavier
University Hospital, I will be able to:

SKILLS (PSYCHOMOTOR)
1. Perform nursing procedures such as monitoring of vital signs and
administration of medications
2. Accurately document interventions and plot vital signs on TPR
sheet
3. Accurately administer medications as prescribed by the doctor with
minimal assistance

KNOWLEDGE (COGNITIVE)
1. Recall and apply the concepts of Nursing process
2. Review the role and responsibilities of the nurse in the general
ward

3. Review and apply the patient’s bill of rights in caring for client

CONTEXT It is the first day of our clinical duties in the
General Ward of Maria Reyna – Xavier University
Hospital Incorporated. We were scheduled for the
morning shift in station 2C. Each of us were
assigned 1 – 2 patients to take care for the rest of the
week or until discharge.

The general ward is different from the surgical


EXPERIENCE ward in the sense that the patients have individual
rooms and some have their own private nurses. My
first patient was a senior citizen and even though he
is quite old, he is still alert, cooperative, and with
good orientation. My second patient was a middle
aged female for discharge. Both of them were in
good condition during our introductory phase.

REFLECTION I have realized that reinforcement of medications
is important for patient compliance, and that timely
administration of medications is crucial I the healing
process of the client. This being said, we have to
explain and let the patient and family understand the
importance of such things in order for healing to
progress smoothly. Further, the responsibility of a
nurse is to take care of their patient’s until such time
that they are discharged from the hospital. As such,
continuous care and monitoring is still part of the
nurse’s role in the hospital.

In order for me to perform my role better, I have


ACTION to review and be familiar with the routine work in the
nurses’ station. I also have to manage my time so as
to not affect the work flow of the staff as well as the
daily activities of the patients. And when charting, I
have to make sure to accurately document the
interventions provided to the patient as well as the
medications.

Overall, the first day of duties were not quite


EVALUATION smooth but I was able to identify the areas that I
need to improve. Our clinical instructor was present
and oriented us well of our responsibilities. The
hospital staff were also accommodating and
approachable which made us feel somehow at ease.

CONTEXT It is the second day of our hospital duty in the
General Ward of Maria Reyna – Xavier University
Hospital Incorporated. We were still scheduled for
the morning shift in station 2C. This day, the ward
was full with admissions of post-surgery clients
while some patients were cleared for discharge.

We were given new sets of patients to take care


EXPERIENCE and all of us were quite busy. My first patient was the
same as yesterday and I believe that we have started
to build rapport with the client and the watcher. He
even recognized me upon entering the room and
greeted actively. I was also able to perform a

subcutaneous injection on him and he was willing
and cooperative. My second and new patient was a
middle aged man who underwent surgery the night
before. He complains about pain from his incision
but other than that he is alert, and oriented to time
and place. This day I feel a bit confident that I am
able to do my role properly than the day before.

I have learned that as nurses we also have to


REFLECTION cater to our patients’ needs. There were a number of
requests that I received from my patients and I was
able to forward them to the nurse assistant with
which I felt satisfied knowing that I was able to
become a channel for communication in providing
their needs. Also, I have observed that one of the
challenges we encounter in the ward was
documenting on time because the patients’ charts
are always borrowed by doctors from the station and
so it would take time for us to do the charting.

Since both of my patients have self-care deficits,


ACTION it is important to emphasize to the watcher that
providing assistance during movement or
ambulation is important to prevent the risk of falling
and minimize further injuries. Medication teaching
should also be reinforced since some of the families
are hesitant to give the medications for fear of
adverse reaction. And as observed, since charting is
a challenge for us, we must practice at home and do
our best to chart on time.

Finally, I can say that this day was better than


EVALUATION yesterday. I and my groupmates arrived and hour
before our duty time and our work flow was rather
smooth compared to the first day. Our clinical
instructor carefully guided us in doing the
procedures, and my groupmates were helpful as we
had good team dynamics.
