Student evaluation
Develop a means for assessing student satisfaction for your program. You will develop a survey/questionnaire to gather information
from students. Develop questions that will be helpful for judging the quality of your program and which will lead to program
improvement. See Chapter 5 in your book for an example.
Description of evaluation
Your evaluation will consist of at least 20 questions, at least 10 Likert questions, 5 rating questions, and one open-ended question.
Additional quality questions will be rewarded with bonus points.
Points to remember:
● Don’t ask for information that you already have
i. I always dress for physical education class
ii. My physical education class is required
● Phrase stems in the form of a statement rather than a question; let them agree or disagree with the statement
● Number all of the questions consecutively in the document (continue from one section to the next)
● Avoid the use of “and”, “but”, or other connecting words that actually make the question ask about 2 different ideas
● Use first person (e.g., parents, “my child”; students “I or we”
● Ask negative questions are those that the low answer is more desirable
i. My child does not like to play golf
ii. Mix the negative items with the positive items; don’t group them all together
● Group like questions together (e.g., ranking questions, Likert questions, open ended questions)
The survey will be evaluated using the following (Remember that we will stop grading after the 5th grammar/spelling error):

6. Physical Education should have an option to pass skill-based test or fitness-based tests.
8. Physical education should only have written tests about gameplay, tactics, & strategies.
9. I will never enroll for another physical education course after they’re no longer required
10. Physical education elective programs are fun with curriculum designed to fit my personal level.
