INFORM CONSENT

We are Senior High School Students from Our Lady of Fatima University and we are doing a research study
entitle:Call Of Duty : An Observation about the Motivation of Firefighters in Valenzuela Philippines. The
study is about the experiences of individual in a Firehouse. Since your school has a SHS STEM Students that
possesses the criteria set by the researcher that can participate to our study. We’re asking for your permission to
answer the questionnaire that was prepared by us and join to this research study.

This study aims to explore and understand the lived experiences inside the firehouse.

This research will require 15-30 minutes of your time. You will be going to an interview having a guide
questionnaire about your experiences inside the firehouse.

There is no risk involved in this study except your valuable time. But the students can understand what was the
feeling and awareness about being a firefighter.

Your participation in this research is completely voluntary. You may also withdraw any time from the study for
any reason. You may also refuse to answer some or all the questions if you don’t feel comfortable with these
questions.

The information provided by you will remain confidential; nobody except the researches will have an access to it.
Your names and identities will also not be disclosed at anytime. However the date may be seen by Ethical review
committee and may be published in journal and elsewhere without giving your names or disclosing your identity.
