Dear Parent/Guardian:

The PWU-JASMS Senior High School Music Students will be participating with the PWU School of
Music at the “TRIBUTE TO THE KING”, a concert honouring the National Artist Lucrecia R. Kasilag, at
the Cultural Center of the Philippines. This is an opportunity for them to experience performing with
the country’s leading artists and topnotch performing groups. They will be joined by four faculty
members of the School of Music. The schedule is as follows:

September 20 – Technical Rehearsals at the CCP Main Theatre (6:00pm to 8:30pm)

September 21 - General Rehearsals at the CCP Main Theatre (3:00pm to 6:30pm)
September 22 - Departure to CCP (12:00NN)
Performance (3:00PM and 8:00PM)

In relation to this, we are requiring that a parent of each participant sign a waiver to signify permission
to join. Kindly sign below to affirm that you are allowing your child to participate in CCP concert.
