CHECK LIST FOR CANDIDATE REQUIREMENTS

Please ensure that you have made available all of the items on the check list seven (7) working
days before the exam date.
This will enable us to process your application efficiently.

 Enrolment form
(duly filled and experience verified by previous/present employer/company)

 Passport copy

 Two passport size photographs (name written at the back)

 Vision certificate
(Successful eyesight test demonstrating that your near vision acuity permits reading a minimum of Jaeger number 1
orTimes Roman N4.5 at not less than 30 cm with one or both eyes,either corrected or uncorrected)

 Fee payment / transfer receipt

 Retest candidates to submit the copy of official result notice

 Renewal candidates to submit the renewal approval letter

 CSWIP 3.2 candidates to submit copy of their valid CSWIP 3.1 certificate

 BGAS candidates to submit experience form.

Failure to provide any of the above will result in additional charges and delays in
examination processing.
