Guide for Teacher I - III Portfolio Preparation:

Things we need:

1. Copy of RPMS Tool appropriate to your level

2. Five (5) long folders or a clear book for five KRAs

3. Binder clips/ fastener

4. Original documents

5. Photocopy of the documents

6. Tabs

7. Highlighter/marker

We need to prepare MOVs every quarter if our goal is to obtain an outstanding or very satisfactory rating
in IPCRF.

For easy organization, you may have a folder or envelope to

contain the same kinds of documents.

For example, you have an envelope containing COT rating sheet

and/or inter-observer agreement form and an envelope containing

your lesson plans or modified Daily Lesson Logs (DLLs).

Label the envelope with the name of the document contained in

it for easy retrieval later.

Remember that one document may be used as MOV or evidence

of performance for other objectives. For example, if you are a

Teacher II, your COT rating sheet may contain rating or feedback

about: (i) knowledge of content (Objective 1); (ii) teaching strategies


that develop critical and creative thinking (Objective 3); and (iii)

differentiated, developmentally appropriate learning experiences

(Objective 6).

Tips:

A. Start gathering documents at the beginning of the school year.

B. Follow the steps in preparing your documents and organizing your Portfolio.

C. Check for the completeness of the documents needed by preparing a checklist.

D. Reproduce the documents in clear copies.

E. Have the photocopied documents in long bond paper

F. Label properly all the documents for easy

reference.

G. Submit complete documents and keep

them intact.

H. Keep your Portfolio simple yet presentable.

The attached file may help you organize the needed documents. Please just omit "6 as COT rating" since
it has been deleted in the updated tool. The highest rating in COT that a teacher can achieve is 7, and
lowest is 3. In the updated version, the following have been added in Objectives 10 and 11:

Objective 10: COT rating sheet about using diagnostic, formative and summative assessment strategies;
and Developed diagnostic tests; (a). with TOS reviewed by superior, (b) with sample accomplished
questionnaire/answer sheets.

Objective 11: Compilation of a learner's written work with summary of results and with signature of
parents.
