AUTHORIZATION AND CONSENT

I, ____________________________________________________________, declare and confirm as follows:

1. I am applying for employment with IBM India Pvt. Ltd. (“IBM India”, which expression shall include, where
appropriate, any third party authorized to act on behalf of IBM India for the purposes contemplated under
this consent form).
2. I understand and agree that, as part of the recruitment process, IBM India shall collect my biometric
information (fingerprints), for the purposes of verifying and validating my identity at a later date if required,
and for other similar authentication purposes.
3. I hereby provide my explicit consent for IBM India to collect, retain, handle and otherwise process such
biometric information for recruitment, verification and authentication purposes.
4. I understand and agree that IBM India shall collect, retain, handle and otherwise process such information in
accordance with its Biometric Information collection and usage policy, a copy of which shall be provided to
me on request.
5. I understand and agree that IBM India may authorize third parties to collect, store, handle, process, or
otherwise use such biometric information for the purposes contemplated under this consent form (including
for any purposes related to, incidental to or necessary to accomplish such purposes), in accordance with
IBM India’s biometric collection and usage and other policies, and explicitly consent to IBM India disclosing
or transferring my biometric information to such third parties, whether located in India or outside India. I
understand that IBM India shall require such third parties to follow levels of data protection similar to that
followed by IBM India, or as required by applicable law. The name and address of any such third parties that
handle my biometric information shall be provided to me on request.
6. I understand that I may seek to review the biometric information provided by me and retained by IBM India.
7. I understand and agree that if I do not give the consents and permissions stated in this consent form, IBM
India will not be able to consider my candidature for employment. I further understand that this authorization
and consent does not guarantee my employment with IBM India.
8. I understand that if I have any queries or grievances related to the biometric information provided by me, I
may contact Recruitment Biometric Queries/India/Contr/IBM
9. I further understand that this authorization and consent is provided solely for the purpose as specified
above. I understand that IBM India shall treat my biometric information disclosed to it as confidential. IBM
India shall use appropriate security standards to keep my biometric information confidential.
