The Subject Teacher provides quality instruction in the classroom and takes an active role in the
holistic development of students in the school.

I. Individual OBPES (Outcome-Based Performance Evaluation System) (15%)


1. Writes Daily Work Journal. (5%)
2. Prepares and submits Evidence Folder. (5%)
3. Submits OBPES monthly. (5%)
II. Main Functions (60%)
A. Preparation (20%)
1. Enhances Unit Plan Stages 1, 2, and 3 of subjects handled for the 4 quarters.
2. Craft Stage 2 in detail for 4 quarters in all subjects handled.
3. Constructs formative assessments. (No. of formative tests: ____)
4. Constructs summative assessments. (No. of summative tests: ____)
5. Makes and submits Table of Specifications aligned with the prescribed
competencies.
6. Constructs and submits Test Questionnaires aligned with the prescribed
competencies.
7. Prepares and distributes the matrix of the Performance Task per quarter.
B. Instruction (23%)
1. Conducts Diagnostic test and Post-test per quarter; submit results with analysis.
2. Submits, implements and enhances the UbD Stages 1, 2 and 3 per quarter.
3. Conducts classes regularly. (No. of sessions: ____)
2.a. Conducts Laboratory classes (for Science, English, TLE, ICT subjects only)

4. Covers all competencies required for each quarter.

1. Provides intervention to students with academic/behavioral problems in his/her
class period. (4%)
2. Confers with parents of students with academic/behavioral problem. (2%)
E. Other Related Task (1%)
1. Does other special task/s that may be assigned and approved by higher
authorities. (NOTE: The task assigned must be performed all throughout the
school year.)

a. Punctuality and Attendance: Reports to office work and completes
