Senior Portrait Order

Hello all SHS Orchestra Seniors!

Traditionally, each year, the SHS Orchestra Seniors prepare a senior gift to give to the music
department faculty. As we are approaching our year-end concert, we are asking that everyone
contributes to our final statement as seniors to officially thank our wonderful music faculty for
their support and guidance over the last four year years.

Our gift will include personalized gifts to Mr. Boitz, Mr. Ford, Mr. Shiuan, and Mrs. Hasegawa,
as well as a framed collection of our senior portraits to the music department.

We are asking for a​ $15 dollar donation​ to help contribute to the total costs of our gift.

Please enclose the cash donation along with a copy of your 2.5” x 3.5” senior portrait in this
envelope and turn it in by Tuesday, April 30.

If you do not have a copy of your senior portrait, we will print one for you! Please check the box
in the upper right hand corner of this form and enclose an ​additional $2 ​to cover the printing
costs.
