Create or identify an incident/ accident and prepare an accident report for it.

A group of boys were playing basketball during lunchtime at the school hall. John ran into Jimmy
and bumped him down on purpose. They both started to argue and Jimmy spontaneously slapped John
across his face for the bleeding he caused on Jimmy’s leg. The situation had elevated and the school’s
dean, Mr. Gopie, was close by and saw the situation and came to calm the two boys. He sent Jimmy to
the school’s nurse to aid his injury whereas he took John to the Dean’s office. As Jimmy finished from
the nursed room he was called to the Dean’s office.

John and Jimmy were asked to write a short report on the incident that occurred. There were
four other boys who witness the event as they too were playing basketball. They were asked several
questions by the dean who took further action to report it to the Ministry of Education.

To prevent an incident like this to ever occur between these to boys again, letters were sent for
the boys’ parents to come to school and be informed about the issue. When the parents came to school,
the incident was discussed and they implemented preventative measures. The boys were warned in the
presence of their parents that if any incident tend to occur between them once more, they would be
suspended as well as banned from certain school events, as a result the incident was resolved.
