AFFIDAVIT OF DENIAL

I, MICHAEL SALONGA REYES of legal age, Filipino and a resident of #182 Cinco Cinco,
Cabanatuan City, after having been duly sworn to in accordance with law is hereby depose and
say:

1. That due to the fact that the name MICHAEL REYES seems to be very common, I
told that there have been several namesakes with criminal or derogatory records
found the Court records which (1) Crim. Case No. 450-SD(02)-Viol. Of Sec. 15 Art.
III with RH 6425, Branch 37, (2) Crim. Case No. 502-SD(02)-Viol. Of R.A. 642 as
amended, Branch 89, Provisionally Dismissed, (3) SD(13)-2898 – Theft, Branch 88
at RTC, Sto. Domingo, Nueva Ecija;

2. That I would like to categorically state herein that I have no pending criminal or
civil cases filed against me by any person or entity and that cases dug up in Court
record are merely namesakes and none of them are actually mine;

3. That further, I wish to refute herein that I am the person with pending court case
as my name is “MICHAEL REYES” and have been a resident of the above address
since birth and have not been a resident of any of the address mentioned in the
attached Court record;

4. That I am executing this affidavit in order to attest to the veracity of the foregoing
circumstances and for the purpose of denying herein that I am the same person
refers to in the said list of MICHAEL REYES’s.
