SUBJECT : Request for Salary Deduction Termination

DATE : December 20, 2018

1. This pertains to the salary deduction occurring on my salary from October

15, 2017 up to this date by AFPMBAI.

2. ICOW, undersigned only applied for loan once sometime on April 30, 2011
but was already paid on March 30, 2013 and currently has no existing loan
application with the above-said loaning agency so there is no reason why a
deduction is being made to my salary. Attached herewith are my pay slips for your
reference.

3. Furthermore, it is requested that the deduction be terminated as soon as

possible.

4. Request favorable approval.
