    consultation with the neurodevelopment pediatrician of his son.

    2. Instead of availing of loans, the undersigned is respectfully requesting your


    good office to approve his application for monetization of thirty (30) days of vacation
    leave credits.

    3. Attach herewith are the development assessment summary report and medical
    certificate issued by the developmental and behavioral pediatrician.
