     From the Municipality/City where the principal place of business is
    located
     Requirements:
    i. Certificate of Business Registration from SEC
    ii. Barangay Clearance Certificate
    iii. Two (2) Valid IDs
    iv. Proof of Address such as Contract of Lease/ Certificate of
    Land Title

    2. Registration with the Bureau of Internal Revenue


     From the Regional District Office (RDO) where the principal place
    of business is located
    i. BIR Form 1903 – Application for Registration
    ii. Certificate of Registration from SEC
    iii. Barangay Clearance
    iv. Business permit from Mayor’s Office
    v. Proof of Address such as Contract of Lease/ Certificate of
    Land Title
     Payment of Registration Form (BIR Form 0605) and Documentary
    Stamp Tax (BIR Form 2000), if applicable
     Registration of book of accounts and receipts/invoices
