Consent Letter

Dear Respondent:
We are Senior Highschool students in 11th Grade of Darwin International
School- Malolos, Bulacan who are currently conducting a research entitled
“Filipinos On Divorce” in our An Introduction to Qualitative Research Methods
and Report Writing Class under Ma’am Liezl F. Velasquez. Our study requires the
administration of interview form which we will derive answers to our research
problem.

Through this letter, we would like to request your voluntary cooperation to


be one of the interviewees. As a respondent, you are expected to answer the
interview questions.

Be aware that the interview will be audio-recorded but rest assured that your
responses to the interview questions will be treated with utmost confidentiality and
your personal details will remain anonymous throughout and after the publication
of the study.

We look forward to receiving your affirmative response to this request of


ours. Be advised that each question must be answered unless it does not permit to
do so.
