Dear Respondents,
Good day! We are the Third-year student of Bachelor of Science in Business
administration Major in Marketing Management, we would like to humbly request your
expertise to validate our thesis entitled: Delivery practices and effectiveness of one
delivery company in Ragay, Camarines Sur: Basis for a service model development.

The objectives of the study are: (1) To determine the delivery practices of the
company; (2) To determine the effectiveness of delivery practices of the company; (3) To
determine the challenges and problems that mostly occurs in the delivery process; (4) To
figure out what possible service model can be proposed to enhance the delivery system of
the logistic company. This research will enable us to understand the processes and
challenges in the delivery systems of the company.

In connection with this, the researchers will be needing assistance with regard to the
validation of their questionnaires necessary for the study. Knowing that you are most fit and
capable to provide such, the undersigned has chosen and we would like to ask approval
from your good office to be the evaluator. Your responses will be a great help to ensure that
the research instrument that we will use for gathering the data needed to accomplish our
