That as an applicant I have complied and submitted all the required documents to the Division
Selection Committee of The Division of Misamis Oriental pursuant to DepEd Order No.42, s., 2007 known as
“The Revised Guidelines on the Selection, Promotion and Designation of School Heads”.

That all records I have submitted are verified, true and authentic;

That I assume full responsibility for any misinterpretation that may uncovered in the future with regards
to such records.

That I am executing this instrument to attest to the veracity and truthfulness of all the foregoing
statement and for whatever legal purpose that this may validly serve.
