AFFIDAVIT OF SERVICE

I, LUIS N. GNILO, of legal age, single, and resident of Phase 2 Block 56 Lot
19 Parklane, Brgy. Santiago, General Trias Cavite, after having been duly
sworn in accordance with law, hereby depose and say:

That I am the liaison officer / Legal Aide of the law firm of Alamia Santiago
and Partners with office address at Don Alfaro St. Tetuan 2nd floor MMS
Building , Philippines;

That in my aforementioned capacity, I tried to effect personal service of a

copy of the Notice of Dishonor upon Mr. Rhenn Mark Bautista on June
20, 2019. But the Security Guard of the said Condominium Mr. Richard F.
Maala said that he already moved out from the address on February 08,
2019.

I am executing this affidavit for whatever legal purposes it may serve.
