As per report and records of the events available in the official police
blotter of this office there appears the following data, to wit:

DATE AND TIME ENTERED : 5 AUGUST 2019; 2:00 PM

FACTS OF THE CASE:

(QUALIFIED THEFT) : One A, of legal age, single, and the President and
CEO of ABC Corporation, with office address at 3rd Floor of the RCBC Building
in Jupiter, Makati City, appeared in this station and reported that several of
his employees have taken company properties for personal gain. The employees
identified were W, X, Y, and Z, all Filipino citizens.

Entry Number 01-082019 / 5 August 2019 / 2:00 PM : Person of A


appeared in this office and reported that the subjects who stole from his
company are W, X, Y, and Z.

Entry Number 01-082022 / 7 August 2019 / 4:00 PM : Initial


investigation conducted by this section at the place of the incident disclosed
that W, X, Y, and Z were employees of ABC Corporation and were in-charge of
managing and maintaining the business’ warehouse where all of the alleged
stolen items were stored. It was identified that the following were the
designations of the alleged subjects - W, Warehouse Supervisor; X, Assistant
Warehouse Supervisor; Y, Delivery Driver and Warehouse Assistant; and Z,
Warehouse Assistant. It was also disclosed that several products and inventory
items were missing, and the said missing items were identified when an
inventory count was made. It was alleged by A that the stealing incidents
happened sometime January 2017 to April 2018 and May 2019.
