I. PURPOSE of INTERNSHIP CONTRACT/AGREEMENT

The purpose of this INTERNSHIP CONTRACT/AGREEMENT is to establish


harmonious collaboration/linkages between St. Vincent’s College Incorporated (SVCI) and
___________________________________________________________________ for an
effective implementation of the policies and guidelines of the professional exposure program and
to support the educational environment of the student intern.

The INTERNSHIP CONTRACT/AGREEMENT aims to achieve the learning


competencies of the professional exposure program namely but not limited to: to develop and
enhance further knowledge, skills, and attitudes of students relevant to the workplace or
profession; improve competence and boost confidence of the students in dealing with day-to-day
realities in the workplace; and to enable the students to relate concepts learned from the
classroom into the workplace.

II. PERIOD and DURATION

The affectivity of this INTERNSHIP CONTRACT/AGREEMENT will be on

III. ROLES, RESPONSIBILITIES and CONDITIONS

Both parties shall adhere the following INTERNSHIP CONTRACT/AGREEMENT


ROLES, RESPONSIBILITIES and CONDITIONS:

A. The FIRST PARTY shall:

1. Attend and complete the agreed duration of his/her internship; In case the FIRST PARTY
will unable to finish his/her internship within the designated period, he/she shall inform
the Practicum Coordinator/Adviser in writing of his/her intent and reasons to prematurely

end his/her internship at least three (3) working days before his/her last day of internship.
Failure to complete the professional exposure program without valid cause disqualifies
the student intern from retaking the program with the SECOND PARTY;
2. Conduct himself/herself in such a manner that will not bring disagree of dishonor to the
SECOND PARTY and to St. Vincent’s College Incorporated;
3. Abide and obey the rules, policies and regulations of the SECOND PARTY relative to
his/her professional exposure program;
4. Protect properties of the SECOND PARTY and solely responsible and accountable for
any loss or damage to any material, tools, equipment, etc. due to carelessness and
negligence of the FIRST PARTY; and
5. Maintain confidentiality, when and where appropriate during and after the internship
period of all the data, business or trade secrets where such information is not within the
public domain and is indicated or understood to be confidential.

B. The SECOND PARTY shall:


1. Design and implement the work-assignment-schedule plan, which will be followed by the
FIRST PARTY;
2. Encourage the FIRST PARTY to develop their personality and professionalism, and to the
extent possible, protect the FIRST PARTY from physical and moral danger;
3. Assign/designate a Practicum Supervisor responsible for the implementation of all the
phases of the professional exposure program;
4. Orient the FIRST PARTY on the standard rules, policies and regulations of the SECOND
PARTY relative to internship/practicum;
5. Provide the practical training or work experience to the FIRST PARTY by providing
necessary tools/equipment, facilities, supplies and materials relative to professional
exposure/practicum of the FIRST PARTY;
6. Assign the FIRST PARTY specific job or tasks relative to the competencies,
qualifications and field of study of the FIRST PARTY;
7. Ensures a safe working environment for students undergoing practicum and provide first
aid treatment in case of work related accidents, however the SECOND PARTY shall not
responsible/liable for any injuries, accidents or damages that the FIRST PARTY may
have caused which be due to his/her own negligence or carelessness while undergoing
internship in its premises;
8. Evaluate the performance of the FIRST PARTY during and after the
practicum/internship, in accordance with the assessment tools provided by St. Vincent’s
College Incorporated; and
9. Issue certificate of completion to the FIRST PARTY not later than two weeks after the
completion of the internship/professional exposure program.

IV. PLACE of ASSIGNMENT

The SECOND PARTY will assign and designate a specific SITE where the FIRST
PARTY can perform his/her professional exposure and practical training. The FIRST
PARTY shall be assign at

IV. INCENTIVE and/or BENIFITS

The SECOND PARTY may provide the FIRST PARTY an incentive in a form of
but not limited to: duty meal allowance, transportation allowance and regular allowance
depending on the corporate social responsibilities.
V. CONFIDENTIALITY

The INTERNSHIP CONTRACT/AGREEMENT, as well as the roles,


responsibilities, terms and conditions herein shall be held in strict confidence and that no
portion or part thereof maybe reproduced or be revealed to any personnel or entity other
that the signatories herein or their authorized representative.

VI. EFFECTIVITY

The Internship Contract/Agreement shall take effect upon signing of both parties
and shall remain in force will otherwise revoked or terminated in writing by all parties.

VII. AMENDMENT

No amendment, alteration or modification of the terms and conditions in this


contract/agreement shall be valid unless evidence by written agreement of both parties. If
for any reason, any part of this contract/agreement shall be held unconstitutional and/or
invalid, other parts of the provisions hereof which are not affected hereby continue top be
in full force and effect.
