AFFIDAVIT OF LOSS

I, JOHN PAULO CRUZ, of legal age, Filipino and a resident of Iba

O’este, Calumpit, Bulacan, under oath, depose and say:

1. That I am the lawful holder of a HOTEL PRO ID issued by the

Company;

2. That on July 4, 2019, at Hotel New World the said ID was

misplaced or left somewhere which I cannot recall despite diligent
efforts to locate it, I cannot find it anymore;

3. That the lost ID was not borrowed by anyone from me for

any unlawful purpose;

5. That I execute this affidavit to vouch for the truth of the foregoing
and for all other legal intents and purposes it may serve.

IN WITNESS, WHEREOF, I have hereunto signed my name on this

instrument on this 9th day of July, 2019 in the City of Malolos, Province of
