SUB: Letter of Undertaking

With reference to the above subject, I would bring to your kind notice, that I was unable to
attend to the college duties regularly due to my health issues on the following days i.e., from 01/09/17 to
25/10/17. In this regard, I request your kind self to consider the above leaves on medical basis and permit
me to resume for the duty from tomorrow onwards (26/10/17). I hereby undertake that I will not go on
leave without prior information and follow the college norms to avail the leaves, in future if I repeat such
incident again you may please take suitable action against me according to university norms. I have
enclosed the medical records with this letter.
