Submit this form to request approval from/notify Gaming Licensing and Development Department (GLDD) of the
proposed betting limits to be implemented at gaming sites, to wit:
a) Operator to request approval from GLDD of the proposed betting limits for electronic table games.
b) Operator to notify GLDD at least one (1) business day prior to implementation of the proposed betting limits for
any game other than electronic table games.
Only requests/notifications with complete information will be processed. Failure to provide complete information could
result in non-processing of this request/notification. Cut-off for submission of requests/notifications shall be 4:00 P.M.

PLEASE ALLOW FIVE (5) BUSINESS DAYS FROM THE SUBMISSION OF COMPLETE INFORMATION AND
ATTACHMENTS FOR GLDD TO PROCESS YOUR REQUEST FOR APPROVAL.
PAGCOR RESERVES THE RIGHT TO CONDUCT POST-AUDIT OF THE IMPLEMENTATION OF BETTING
LIMITS AT GAMING SITE.

Attached as Annex A is the updated list of betting limits which is inclusive of the proposal stated in Section
C.
We confirm and undertake to do the following:
 Proposed betting limits for table games shall have approval of GLDD prior to implementation;
 Proposed betting limits for any game other than electronic table games shall be notified to GLDD
prior to implementation; and
 The abovementioned undertakings shall be carried out in close coordination with GLDD, and
Compliance Monitoring and Enforcement Department (CMED).

copies may be submitted directly to the GLDD office or scanned copies of the documents may be emailed
