    ng social media accounts and bank account registered sa contact number na yun?
     
    Hassle pag kukuha ka ng bagong number, and then iuupdate lahat ng accounts sa
    new number.

    AFFIDAVIT OF LOSS

    I, ______, Filipino, of legal age and a resident of _______________________, after


    being duly sworn in accordance with law, depose and state that:

    That I am a regular user of a Globe Sim card with no. of ___________ since 201__;

    That on March 29,2019, at about 7:30 to 8:30 in the morning when I left the
    premises of the Alabang-Sucat Route Bus that I usually ride going to work, I lost my
    J7+ cellphone where the Globe sim card is inserted;

    A few minutes after noticing that my cellphone is not with me, I returned to the
    Sucat drop off area to see if the bus is still there;

    I failed to recover my cellphone and the said SIM Card despite diligent search and
    earnest efforts to contact and locate said phone, and I consider the same as lost and
    can no longer be found;

     
    As such, I am executing this Affidavit of Loss to attest to the truth of the
    foregoing and in support of my request for the replacement of the said SIM Card
    and for whatever legal purpose it may serve.

    Affiant states nothing further.
