AFFIDAVIT OF LOSS OF ATM CARD

I, VICTOR DENNIS T. NGO, Filipino, of legal age, single, and a

resident of Unit 309B Cluster 7, Makati Prime City Condominium, St. Paul Road,
Makati City after having been duly sworn in accordance with law, hereby depose
and say:

1. That I am a holder of the BDO-ATM card with account number 4120

160 168 issued by BF Homes President Ave. Branch;

2. That I used to put said ATM card in my wallet;

3. That on September 4, 2018, I withdrew using my BDO-ATM card in BDO

Ortigas Branch an thereafter I thought I placed the same in my wallet;

4. That sometime on September 5, 2018, when I was about to withdraw in the

morning, I checked my wallet and to my surprise my BDO-ATM card was not
there. I reported the loss immediately though BDO hotline.

5. That diligent effort to locate the same had proved futile;

6. That I am executing this affidavit to attest to the truth of the foregoing facts
and to request for a replacement of my BDO-ATM card.
