Documentary Requirements

1. Duly Accomplish and Notarized application Form

2. Latest Certified true copy of Mayors Permit
3. Proof of ownership or Lawful occupancy of principal head office of business (i.e., electric, water or telephone line bill)
4.
5. Secretary Certificate, certifying that a board resolution was passed authorizing the key officer / Board member indicated
in the GIS to sign the application with his/her specimen signature, picture and photocopy of valid government issued
identification card.
6. Latest Mayors Permit CTC
7. Tax Clearance issued by your RDO
8. Delinquency Verification certificate issued by the Regional office
9. Certificate of Good Standing issued by SEC (original)
10. Enrolled as EFPS user

Securing the Requirements

ITEM 2. Proof of billing (latest billing of MERALCO or WATER)

ITEM 3. Board Resolution duly signed by the corporate secretary.

ITEM 4. CTC of Mayors Permit

A. Requirements: Original Mayors Permit, Official receipt for mayor permit payment, photocopy of mayors permit and the
official receipt.
B. Go to Licensing department
C. Present the above required documents
D. Pay the corresponding payment at the miscellaneous
E. Go back to licensing department and wait for the release.
