    Subject: DISCONNECTION OF PTCL DSL (2-Mbps) INTERNET CONNECTION

    With reference to captioned subject, the undersigned desires to disconnect the DSL 2-Mbps
    Internet Connection against the Tel # 06858-75985 at House No. 4/A New Officers Colony, RYK with
    immediate effect. Only voice package will be used by undersigned in the future. For disconnection
    purposes, the DSL Device along with requisite documents is also returned herewith.

    It is therefore requested to disconnect the DSL internet connection before next month billing please.
