Instructions:
Step 1. Prepare School Calendar of Activities, Project Proposal for the different activities or
projects to be conducted and celebrated (Nutrition Month, Quarterly Test with Item
Analysis and Least Learned Skills)

Step 2. Submit to the principal, EPS and SDS for approval.

Step 3.Conduct, administer or launch the planned activities as scheduled.

Step 4. Prepare narrative reports, consolidated reports conducted, administered and

launched activities.

Step 5. Submit a hard copy of accomplished reports to the principal

Step 6. Submit the hard copy of accomplished reports to the EPS and SDS for approval and
evaluation

Step 7. Submit a Year-End Accomplishment Report to the principal and EPS in EPP for
evaluation and verification of the different activities conducted the whole-year round.

Step 8. Compile by process
