RELEASE, WAIVER

AND QUITCLAIM

the amount of _______________________ from D & R FAST AUTO REPAIR SERVICE
as full and final settlement of any and all claims which I may now or in the future have with
or against my employer D & R FAST AUTO REPAIR SERVICE, its officers and employees
as a consequence of my voluntary resignation from said company.

In consideration of the above-mentioned payment, I discharge and, by these presents,

for myself and my heirs, successors and assigns, release and discharge, my employer, D & R
FAST AUTO REPAIR SERVICE its representatives, agents, successors and assigns, from
any and all cause or causes of actions, sum or sums of money, accounts, damages, claims and
demands, in law or in equity, which I had, now have or which I, my heirs, successors and
assigns shall have against D & R FAST AUTO REPAIR SERVICE.

I am executing the foregoing Release Waiver and Quitclaim willingly and voluntarily
and with full knowledge of my rights under the law.
