RELEASE WAIVER AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

City________________________________, voluntarily acknowledge having received from
SKILLED IN RECRUITMENT AND OUTSOURCING SERVICES INC., the amount of
_______Two tousand, two hundred _________________________ pesos (Php
___2,200__________) as full and complete settlement of all my remunerations, claims,
and other benefits which have accrued to me or to which I am entitled under the law by
reason of my past services with Skilled in Recruitment and Outsourcing Services Inc.

In consideration of which -

1. I unqualifiedly acknowledge the legality and validity of the severance of my

employment with Skilled in Recruitment and Outsourcing Services Inc.

2. I release and forever discharge Skilled in Recruitment and Outsourcing Services Inc.
and its successors-in-interest, stockholders, officers, directors, agents or employees from
any action, sum of money, damages, claims and demands whatsoever, which in law or in
equity I ever had, now have, or which I, my successors and assigns hereafter may have by
reason of any matter, cause or thing whatsoever, up to the time of these presents, the
intention hereof being completely and absolutely to release Skilled in Recruitment and
Outsourcing Services Inc. as well as its successors-in-interest, stockholders, officers,
directors, agents or employees from any and all liabilities arising wholly, partially or directly
from my past services with Skilled in Recruitment and Outsourcing Services Inc.

3. I acknowledge that I have received all amounts that are now, or in the future, may
be due me from Skilled in Recruitment and Outsourcing Services Inc. I also acknowledge
that during the entire period of my employment with Skilled in Recruitment and Outsourcing
Services Inc., I received and was paid all compensations, benefits and privileges to which I
was entitled under all laws, and if I hereafter be found in any manner to have been entitled
to any amount, the above consideration is a full and final satisfaction of any and all such
undisclosed claims.

4. I also manifest that the payment by Skilled in Recruitment and Outsourcing Services
Inc. of any or all of the foregoing sum of money shall not be taken by me, my heirs or
assigns as a confession and/or admission of liability on the part of Skilled in Recruitment
and Outsourcing Services Inc., as well as its successors-in-interest, stockholders, officers,
directors, agents or employees for any matter, cause, demand or claim that I may have
against any or all of them.

5. I further warrant that I will institute no action and will not continue to prosecute any
pending action, against Skilled in Recruitment and Outsourcing Services Inc. by reason of
my past employment with Skilled in Recruitment and Outsourcing Services Inc.

6. I will not at any time and in any manner whatsoever, directly or indirectly, disclose
to any person or entity any information of any kind relating to Skilled in Recruitment and
Outsourcing Services Inc., and the business of Skilled in Recruitment and Outsourcing
Services Inc., including their trade secrets, manner of operations, plans, processes or data
of any kind.

7. I finally declare that I have read and fully understand this document, and the
release, waiver and quitclaim hereby given is made willingly and voluntarily and with full
knowledge of my rights under the law.
