RELEASE, WAIVER AND QUITCLAIM

1. That for and in consideration of the sum of PESOS: ________________ (P ), to me

settlement of all claims, receipt of which is hereby acknowledged to my complete and full
satisfaction, I HEREBY RELEASE AND FOREVER DISCHARGE the above-named
corporation from any and all claims arising from my previous employment with the above-

2. With this receipt, I hereby certify that all of my claims against the above-named
corporation have been fully paid to my complete and full satisfaction. My intention herein is to
completely and absolutely discharge and release the corporation, its stockholders, officers,
directors, representatives and agents from all claims and liabilities arising from my
employment with said corporation.

3. The above-named corporation’s payment of the aforementioned amounts shall not be

taken by me, my heirs or assigns as a confession and/or admission of liability on its part, and
with regard to the corporation, its stockholders, officers, directors, representatives and agents
for any matter, cause, demand or damages I may have against any and all of them arising out of
or in connection with the aforementioned payments and any other benefit whatsoever arising
from existing laws and company practices and policies.

4. I finally declare that I have read this Release, Waiver and Quitclaim and have
consented to it willingly and voluntarily, and with full knowledge of the meaning and intent of
this document and of my rights under the law.
