The Questionnaire on School Maladjustment Problems has been designed for the
educational context as an instrument to assess the diversity of maladjusted behaviors which
occur in the classroom. The more problematic behaviors can be identified at the class level,
overall (if the questionnaire is completed with the whole class in mind, noting whether these
behaviors appear within the whole class group), or at the individual level (student by
student), identifying which types of behaviors each student presents. The best way to assess
the phenomenon of maladjustment problems is to answer the questions honestly, so that
intervention can be aimed at improving or modifying inappropriate behaviors.

A series of statements having to do with the most common maladjusted behaviors


that take place in educational contexts is shown below. You will find several statements
referring to the behavior and/or attitude of students, which we limit to the last 15 days. In
order to indicate to what extent you agree with each statement, circle the letter that best
matches the situation:

A = Never (this behavior has not happened at any time in the last 15 days).
B = A little (this behavior has happened once or twice in the last 15 days).
C = Some (this behavior has happened 3 or 4 times in the last 15 days).
D = Often (this behavior has happened 5 or more times in the last 15 days).
EXAMPLE

Refusing to do the assignment A B C D

Here the option selected was SOME, indicating that this behavior took place in the
classroom three or four times during the last fifteen days, as shown by circling the letter C.
If you make a mistake you can erase your answer or cross it out and circle the correct letter.
This test has no time limit.

The Questionnaire for the Assessment of School Maladjustment Problems has been
