Dear Mr. Futre,


Relative to your alarming attendance, you were given a memo (May 10, 2019) to explain yourself / justify
your absences.

In your explanation letter, you admitted the said allegation and asked for consideration. Stated further that
you had a hypertension accompanied by medical certificate.

However, said medical certificate did not state that you need to rest or as to how many days of rest. In
addition, you failed to notify your immediate supervisor or seek permission in advance. You only notify the
management regarding this matter after you incurred the absences.

Be advised that per company policy, three (3) days absences in a calendar month carries a penalty of seven
(7) days suspension. Notifying your immediate supervisor whenever an employee cannot report to work is a
responsibility of all employee.

Thus, the management is compelled to penalized you for one (1) DAY OF SUSPENSION WITHOUT PAY this
coming June 26, 2019 (Wednesday).

Let this memo serves as a reminder and a warning that repeated act/s of similar nature shall be dealt with
severely including dismissal from service.

Your resumption of work will be on June 27, 2019.
