D. MECHANISM OF ACTION:
Ranitidine is a competitive inhibitor
of histamine H2-receptors. The
reversible inhibition of H2-
receptors in gastric parietal cells
results in a reduction in both gastric

acid volume and concentration.
Ranitidine’s acid-lowering effect is
more pronounced for basal and
nocturnal acid secretion than it is
for food-stimulated acid secretion.

E. ROUTE/DOSAGE:
Active Duodenal or Gastric ulcer:
(1MOS- 16YEARS)
• For tx: 4-8mg/ kg PO bid.
Maximum of 300 mg a day.
• For maintenance: 2-4 mg/kg
PO once a day. Maximum of
150 mg per day.
• IV: 2-4mg/kg day IV in 6-8 3mins
hrs divided doses. Maxmum:
50 mg/dose or 200 mg/day
• GERD or erosive
esophagitis
term neonates: 2-4
mg/kg/day PO in 8-12 hrs
divided doses or 2 mg/kg/day
IV in 8 hourly divided doses.

• 1mos – 16 years: Oral: 5-


10mg/kg/ day PO divided into
bid. Maximum 300 mg/day.
• IV: 2-4 mg/kg/day IV in 6-8
hourly divided doses.
Maximum of 50-200mg/day;
alternatively, 1mg/kg/dose
infusion once follomg/kg/ day
continuous infusion.

G. ADVERSE EFFECTS :
• diarrhea

I. PREGNANCY CATEGORY:
 Pregnancy Category B
J. INTERACTIONS:
 Delavirdine: Do not take
delavirdine with
ranitidine. Doing so can
cause dangerous effects.
Ranitidine reduces the levels
of delavirdine in your body.
This means delavirdine 5mins
won’t work as well.
 Procainamide: Taking high
doses of ranitidine with
procainamide can cause side
effects from procainamide.
 Warfarin: Taking ranitidine
with warfarin may increase
your risk of bleeding or
blood clots. Your doctor may

watch you more closely if you’re
taking these drugs together
 Midazolam and
triazolam: Taking ranitidine
with either of these drugs
raises your risk of extreme
drowsiness that can last a
long time.
 Glipizide: Taking these
drugs together can increase
your risk for low blood
sugar. You may need to test
your blood sugar or test it
more often when starting or
stopping ranitidine.

K.NURSING IMPLICATIONS:
Assessment
◍ History: Allergy to ranitidine,
impaired renal or hepatic
function, lactation, pregnancy
◍ Physical: Skin lesions;
orientation, affect; pulse,
baseline ECG; liver evaluation,
abdominal examination,

normal output; CBC, LFTs,
renal function tests
◍ Interventions
◍ Administer oral drug with
meals and at bedtime.
◍ Decrease doses in renal and
liver failure.
◍ Provide concurrent antacid
therapy to relieve pain.
◍ Administer IM dose undiluted,
deep into large muscle group.
◍ Arrange for regular follow-up,
including blood tests, to
evaluate effects.

Teaching points
◍ Take drug with meals and at
bedtime. Therapy may continue
for 4–6 weeks or longer.
◍ If you also are using an antacid,
take it exactly as prescribed,
being careful of the times of
administration.
◍ Have regular medical follow-up
care to evaluate your response.
◍ You may experience these side
effects: Constipation or diarrhea 5mins
(request aid from your health
care provider); nausea, vomiting
(take drug with meals);
enlargement of breasts,
impotence or decreased libido
(reversible); headache (adjust
lights and temperature and avoid
noise).
◍ Report sore throat, fever, unusual
bruising or bleeding, tarry stools,
confusion, hallucinations,
dizziness, severe headache,
muscle or joint pain.
