Welcome to Medicine 2.

This posting is designed to help you build your skills in history taking, physical examination and
making a clinical diagnosis.

You are expected to be present in the ward at 9 30 AM. You are expected to work up one patient
in detail, every day. The case for clinics will be posted by Registrars in turns as mentioned below.

You are expected to be regular to the classes and the names of the absentees are to be given to the
Medicine 2 secretary, after informing the consultant concerned.

You are encouraged to read the following books during this posting:

1. Hutchison’s Clinical Examination,

2. Macleod’s Clinical Examination
3. Bate’s Clinical Examination

You are expected to bring the necessary instruments. A record book with case recordings of 15
patients is to be submitted, before the end of posting. This will be marked as a part of internal
assessment

I Clinical 2nd Posting:

I. Objectives:
a. Take a proper and detailed history. (The student should be able to elicit details of common symptoms)
b. Perform a complete and thorough physical examination and proper technique of elicitation
of clinical signs and their interpretations. (to interpret the significance of abnormal signs).
c. Synthesize clinical history and physical examination and do a problem listing and come
to a basic clinical diagnosis.
d. The discussion during the clinics will be based on the above quoted clinical medicine
books
d. Understand the pathophysiology of symptoms and signs and analyze symptoms.
e. To be able to write a case sheet of history and physical examination
f. To develop patient doctor relationship, via good communication skills.

II. Learning skills:

History and complete physical examination of the different systems
Refine clinical examination skills of the student
 Inpatient Care Ambulatory care/OPD(syndromic approaches)
1. Consolidation Diabetes
2. Pleural effusion Hypertension
3. Valvular heart disease Acute febrile illness
4. Congestive heart failure Hepatitis/gastroenteritis
5. Hepatosplenomegaly Respiratory infections
6. Hepatomegaly COPD / Asthma
7. Splenomegaly Tuberculosis
8. Ascites with/without pedal edema Anemia
9. Above with/ without anemia/ jaundice/ Back pain / polyarthritis
lymphadenopathy Upper abdominal pain syndromes
10. Cirrhosis with/without portal hypertension or Altered Bowel habits
encephalopathy Headache
11. Stroke syndromes
12. Meningitis

III. Assessment plans:

Practical exam:
1. Continuous assessment at the bedside by direct observation of physical examination skills
after student has taken complete clinical history.

2. A long case examination at the end of the posting, which would include marks for (eg. out
of 100)

Diagnosis with at least one – two differential diagnosis (10)

Theory: Based on the anatomy and physiology of the symptoms discussed during the clinics
(Reference book is Hutchison)
