Dioscoro S. Cabarrubias, Elesio S. Cabarrubias and Erico S. Cabarrubias, of legal
ages, married, Filipino, with residence and postal address at Back Rubber World,
Bgy. Basak, San Nicolas, Cebu City, do hereby name, constitute and appoint,
Gemma C. Singson, of legal age, Filipino, with residence and postal address at the
above-given address, to be our true and lawful attorney-in-fact and in our name do

“To claim the SSS Death Benefit of our late father

Gilberto S. Cabarrubias who died on April 13, 2012”
