In preparation with the incoming Information Drive regarding the above-mentioned

topics to be conducted by the Leyte Division Health Unit on January 30, 2019, the Grade 9,10,
11, and 12 advisers are required to supervise their students who are the main recipients of the
symposium. Below is the schedule of the Grade Levels who will be attending the said
information drive.

Schedule
Grade 9 & 10- 8:00 AM to 12:00 NN
Grade 11 & 12- 1:00 PM to 5:00 PM

1. Pursuant to the mandate of the CID DepEd Leyte Division on the use of Daily Lesson Log for
all Teachers who have been in the teaching service for 3 years and above, the attached herein
format shall serve as our official template. This maybe compiled in a folder or written in the
Lesson Plan Book using the same format.

2. New Teachers (Less Than 3 years) shall write the plans in the Lesson Plan format.

3. Department Heads and Master Teachers are expected to extend assistance.
