AFFIDAVIT OF DISCREPANCY
(ONE AND THE SAME PERSON)

I, MARVIEY KATE D. OROZCO, Filipino citizen, of legal age, with address at 116 P.
SEVILLA ST. BET. 4TH & 5TH AVENUE, GRACE PARK, CALOOCAN CITY after having
been duly sworn to in accordance with law hereby depose and state:

1. The Birth Certificate issued by Philippines Statistics Authority indicates my name as

MARVIEY KATE D. OROZCO.

2. On the other hand, in Social Security System SSS ID, my name is indicated as
MARVIEY KATE OROZCO without my middle initial. This happened because I have
overlooked to encode my middle initial at their website during registration.

3. Both documents pertain to one and the same person.

4. I am executing this affidavit to attest to the veracity of the foregoing and for whatever
legal purposes it may serve.
