Subject: Maternity Leave Letter

Dear Mr. Thomas,

I would like to request for a two months’ maternity leave, commencing from May 30, 2013. I am
an 8 month pregnant and my delivery is due in the mid of April. My tentative date of joining would
be July 01, 2013. But in case of any impediment, I will update you on priority about the extension
of my maternity leave.

I have attached all the maternity details, including the required medical forms, applications and
other details. Please find them enclosed and let me know if anything is missing. I would like to
avail the entire maternity leave benefits as per our company’s maternity policy.

I understand the front desk’s role in the day to day activities of our company and my absence for
longer period of time will require you to appoint a temporary front desk officer. I have already
completed all the pending tasks and arranged the necessary folders properly. However, I am
more than happy to train the substitute front desk officer during the transition.

I would be really thankful if you kindly confirm my maternity leave, tentative date of joining and
the medical benefits.
