AFFIDAVIT OF CESSATION OF BUSINESS

I, BERNADITA A. LUEGO, of legal age, widow, Filipino and a resident of

Brgy. Ichon, Macrohon, Southern Leyte, after having been duly sworn to in
accordance with law do hereby depose and state THAT:

1. I am the bonafide owner or proprietor of the business with

registered name JUN ARIS;
2. I am executing this Affidavit of Cessation of said business in my
said capacity;
3. I am executing this Affidavit of Cessation to formally close down
my said registered business which has already stopped its operation since the
month of March 2018;
4. Said business has no outstanding liability and obligation;
5. Instant affidavit is duly executed by me whatever legal purpose this
may serve;
6. I executed voluntarily this foregoing Affidavit in order to signify the
truthfulness of the same in my own free will and volition.
