Personal Protective Equipment (P.P.

E) Register

01 WHAT IS A P.P.E REGISTER?

A Personal Protective Equipment (P.P.E) Register logs the issue of personal protective equipment
such as goggles, sunscreen, gloves, safety vests, etc, to each employee.
The Register can be either individual forms for each separate employee, or a master copy with all
employees listed on it.
With any specialty item of P.P.E, such as a harness, there should also be a systematic procedure
to show proper training in the correct use of the equipment has been conducted. This may even be
documented on a Toolbox Talk Form.

The P.P.E register should document:

✓ The name of the person receiving the P.P.E items
✓ The P.P.E received
✓ A signature of the person that has received the P.P.E
✓ A reference that states that the person issued with the P.P.E understands that they must
request a further issue of P.P.E once the item received is no longer useable.
Personal Protective Equipment (P.P.E) Register
List all types of P.P.E that you have been issued with, and trained in their use, and sign and date as your confirmation of receipt.

Signing also confirms you understand that you are responsible for maintaining the equipment and will request new items once the current items are no longer
effective.
