1 PURPOSE.
To ensure that appropriate action is taken to minimize the risks, to protect the personnel and
surrounding environment and to reduce the impact of personnel injuries or losses of equipment.

1. SCOPE.
This procedure covers the scope of works that will be carried out under PT. Dua Pilar Bangunutama.
This Emergency Response Plan addresses emergencies involving all activities such as:

3. Office work.

5.ROLES AND RESPONSIBILITIES.


All personnels are required to ensure that they fulfil their responsibilities during any emergency
through actively participating in their prescribed activities. The following provides a description of
the emergency response responsibilities and activities for Barge Master as the Leader and
individuals.
The following list describes the key elements of the Emergency Response Plan and those responsible
for ensuring adequacy and effectiveness.

5.1. CSR
He will give assistance based on CTR request, for any MEDEVAC decision. He will ensure that
its personnel are ready for operation. Paramedic onboard for any medical case and regularly
inform the up to date situation, prior to the evacuation of the injured person to the nearest
hospital.

5.2. Barge Master.


In case of emergency, Barge Master has responsibility to initiate a Medevac after consultation
with the Paramedic on site. He also has an authority to stop work and instruct to abandon
barge/boat in case of evacuation. He will coordinate with CSR on board to request any
assistant to CPY during emergency situation.

5.3. Safety Officer.


The Safety Officer as Field Commander in ERT possibility to initiate a Medevac when Barge
Master is absent/unavailable.
RESPONSIBILITIES ACTIVITIES

5.4. Paramedic / First Aider.


Paramedic works in emergency medical care, or the care of patients for conditions that
require immediate or very prompt medical attention. Paramedic primarily works as part
of the emergency medical services, providing advanced levels of care for acute medical
problem, but also leads the first aider.

5.5.ERT MEMBER.

6.EMERGENCY RESPONSE PLAN – MEDEVAC.
6.1.Basic Emergency Response Rules.
Immediately respond of calls to the dedicated emergency telephone number or to be addressed to
PT Indo Straits Tbk. Management on site.
Emergency Response Team attends the injured person; secure the area from any potential cause
for secondary injury, or other emergency situation (equipment damage or fire). For injured or
illness, Paramedic on WB Straits Fortune Clinic effort to stabilize the victim before transportation
that will bring patient to hospital come.
Portable Fire Extinguisher units must be present wherever potential fire accident may exist.

7. IMPLEMENTATION
7.1.On Hearing the Site Emergency Alarm
7.1.1.Immediately stop to work and walk to Muster Point (MP). For those who are performing
Hot Work activity shall turn off all the power or secure the welding equipment before walk to
muster point and who are performing Lifting activity shall lower the material first before walk to
muster point. And who are performing confined space operation, the attendance must
immediately instruct all entrants to come out and walk to muster point.
7.1.2. Do not smoke or use any electric equipment such as hand phones, radios etc. That are
not intrinsically safe.
7.1.3.Congregate at the muster point in an orderly manner and take direction from the Barge
Master or Safety Officer.

7.1.4.Remain at the Muster Point until the “All Clear” signal is given.

7.1.5.Do not direct return to work until instructed by Barge Master or Safety Officer that
explains condition is safe to do so or after renew the Permit to work.
7.2 Stabilization at Site
In case of personnel injured, stabilization on Site is done by Paramedic/First Aider at Clinic.
7.3. Alert from Site
All report of emergency condition shall be reported by Barge Master to PHM.
Who to Alert? Refer to FLOWCHART FOR MEDEVAC AT WB STRAITS FORTUNE.
7.4. Information to be provided to PHM and hospital if emergency condition occurs.
The following information is essential upon both the first call and the first written report :
 Name and age of patient(s)
 Nationality of patient(s)
 Location of patient(s) and his travel documents (passport)
 Description of the medical problem:
o What happened
o When did it happen
o How did it happen
o What is the patient complaining of (diagnosis)
o What can be seen to be wrong with the patient
o What treatment has the patient had so far
o Name and phone number of the caller
7.5 Evacuation and Transportation.
Transportation during evacuation will be provided by PT Indo straits Tbk. to nearest dedicated
hospital or other external emergency provider.
In case PT Indo straits Tbk. cannot provide transportation, PT Indo straits Tbk. will ask PHM for
assistance.
7.6. Communication System.
To support communication system during emergency condition, in barge is completed by
telephone, radio, and email.

8.EMERGENCY SCENARIO.

 Emergency scenario is an outline of the action of an event. As used in forecasting and


sequence analysis, scenarios are sequences of events that lead to some specified
circumstances. During in processing area on the near-shore and offshore operation, there are
some emergency scenario that could anticipated and examining condition in case of may be
such as gas leak, vessel collision, fire, man over board and oil spill.

 In case of emergency condition cannot be handled by PT Indo Straits Tbk, assistant such as
clinic, medic, emergency intervention team, transportation, etc shall be asked to PHM.
8.1 Gas Leak.
Barge master commands to sound the emergency alarm and stop all work and activities.
Also instructs to all crew to turn off all ignition source and spark potential and then immediately
assemble at muster point for head counting. Barge master notify Emergency Contact Number on
nearest site what happen and the location and then leads the evacuation (abandon barge).
8.3. Fire / Explosion.
8.3.1. Case by PT Indo Straits Tbk:
Barge master commands to sound the emergency alarm and instructs to ERT Fire Fighter
to extinguish the fire with available fire extinguisher or hydrat and then notify PHM
Emergency Contact Number on nearest site as preliminary report. If the fire uncontrolled,
barge master notify PHM Emergency Contact Number (nearest facilities) for assistance
and then lead the evacuation (abandon barge/boat).
8.3.2. Case Fire at PHM Facilities:
Barge master commands to sound the emergency alarm and notify PHM Emergency
Contact Number on nearest site (nearest facilities) what happen and the location, then
lead the evacuation (abandon barge/boat).
8.5. Oil Spill.
8.5.1. Case by PT. Indo Straits Tbk Activity:
Barge master instructs to sound the alarm and then command spill responder for apply oil
boom surrounding the spill and apply absorbent pads, then barge master notify PHM
Emergency Contact Number what happen and location. In case of major spill barge
master notify PHM for assistance.
8.5.2. Case by PHM Facilities:
Barge Master commands to sound the alarm and then instruct to stop work activity and
turn off all ignition source. Barge master notify PHM Emergency Contact Number (nearest
facilities) what happen and the location. All crew assemble to muster point for head
counting and instruction. If necessary barge master instruct to all crew to abandon the
barge/boat.
Note : In case Medevac requires the victim to be sent to the company’s referral hospital in
Balikpapan. Then contractor will ask for help to the company to provide helicopter. All
medevac fees and treatment cost at the company's referral hospital will be paid by the
contractor (attachment 10.19

9.INSPECTION, TRAINING AND DRILL.


9.1. Inspection of emergency condition equipment is done regularly to ensure that the equipment is
in good condition and ready to be used.
9.2.Specific training shall be given to ERT member such as first aider, fire fighter, etc.
9.3. Emergency condition drill is performed regularly with various scenarios.
