A. SCOPE OF WORK

1. TERMITE SOIL POISONING (SP)

Termite soil poisoning shall be done around the building as close as possible to
the base of the outside walls, either by cordoning, trenching or injection
methods, SP services make the soil inhabitable for subterranean termites,
control any present infestation and discourages future subterranean termites.
Either liquid or powder termiticides shall be used.
Frequency: One (1) General Treatment
B. TERMS AND CONDITION

1. For the services described above, we quote you the amount of Thirty Seven
Thousand Seven Hunred Sixty Pesos only the cost of the total job.

2. JBD Pest Control Services will provide all chemicals, equipments, labor and
supervision to complete the comprehensive Termite Control program.

3. Honest, skilled and highly trained service technicians will be assigned to the job
site to assure you of proper application and treatment.

C. CHEMICAL TO BE USED

1. Only safe, synergize and effective formulation approved by the Food and
Drug Administration (FDA) shall be used for the above-mentioned
project.
