Annual Pest Control Services Contract Renewal (Apr-20 to Mar-21)

Dear Sir,

We, at C-SHINE Group, use biological, genetic and cultural management in rendering our
Pest Management Services, with a focus on eliminating the use of harmful pesticides.
Our aim is to ensure a safe and pest-free environment to our clients, along with
supervision by our trained Pest Control Operators.

Please see below for details of the service that we want to offer to you.

As per our representative detailed meeting and visit at your site, Please find below our
proposal for Pest control Services implementation at your site for your kind
consideration. We recommend using latest pest control devices (Bait Boxes and
Transparent Glue boxes & Mechanical Traps as latest EPA &
Safety Standard) in this area.

Schedule of Services

Our PCO (Pest Control Operator) will visit all of your sites
02 days of week & perform Pest control activities and make your area pest free.

1.1. Rodent (Mouse) Control (Elimination & Monitoring)

Rodent Control will cover all types of rodents through temper resistant Glue
Boxes & Glue boxes for all areas. Frequency for the servicing of Rodent devices
will be alternate day basis
Following strategies would be adopted:

a. Zinc Treatment in your outer adjacent boundaries (if needed)
b. Rodent Mine Elimination

Rodent Prevention & Control Strategy®

c. Three Defense Lines with Mechanical Traps & Glue Boxes & Bait Boxes.
d. Quarterly Strategy Evaluation Audit & Report
e. Weekly & Monthly log sheets
f. Detailed Placement design (Outlet Maps)

1.2 Spray Services (Elimination & Monitoring) Flying and Crawling Insects Control

(A) Rotational Spray of Pyrethroids (Delta-methrine, Permethrin or

Organophosphate) throughout theare
only outside and around your site.
(B) Generally during the season Delta-methrine & alphacypermatrine fumigation would
be used at site.

The concentration of application would be as per label within a range

of 1.00 to 1.50 %per 100 m2. Frequency of Spray is on alternate days or
as per required.

1.3 Lizard Control

Monthly night capturing operations will also be carried out to catch lizards.
These night operations are a manual activity and will be done once in a month.

1.4 Insect Light Trap Monitoring

All insect light traps, provided by the client, will be monitored on monthly basis, which
will help us to analyze the threshold level of flying insects inside the area, and make
corrective action plan accordingly. Provision, repair and maintenance of Insect Light
traps will be responsibility of Client.

2.0 Assistance during External Audit

Comprehensive assistance will be provided prior and during your external audits. Our
expert will visit the site and support you in all kind of documentation according to AIB
Food Safety / BRC requirements.
3.0 Contract Validity

The contract period will be for one year and extendable with mutual consensus. A
minimum 10% increment will be provided by the client on every renewal after every year
with mutual consensus.

4.0 SERVICES & SERVICE FEES

The entire above mentioned Pest control Services at your place will cost you including all
above mentioned services will be including services sales tax.

5.0 Emergency Services / Visits

In case of any Audit or any complaint received from clients, emergency visits will be
plan on immediate basis without extra charges.
