Dear Sir:

For your General Pest Control Maintenance Requirement for the New Phase II Buildings and area
specifically to control flies, mosquitoes, cockroaches, ants, bugs, lizards, termites, birds, snakes and
other flying and crawling insect pest and to include rodent eradication and control. Located at KAIEZ,
Barangay Kamanga, Maasim Sarangani Province. Below, we are pleased to offer you our services:

Phase II areas to be Treated:

Scope of Works:

a. Residual Spraying to prevent and control insect pest such as flies, ants, cockroaches, mosquitoes,
lizard and other flying and crawling insects using environment friendly, biodegradable with low
mammalian toxicity chemicals of BAYER products such as; Temprid, SC, Premise TC, Agenda for
termite prevention and control, Maxforce Forte & Quantum Gel for cockroach and ant control,
Racumin Paste and Racumin Powder for Rodent Control. We are also going to use the product of
Syngenta for alternative use like, Klerat for rodent, Icon EC/SC for vermin control. We also have
Ensystex Product as another alternative like; Delta Thor Plus & Cyflux EW, Vectocide & Stargard
for Vermin control, MaxxThor for termiticide and insecticide use. These products are Approved
and registered with Food and Drugs Administration (FDA) and Fertilizer and Pesticide Authority
(FPA) for Household and Industrial use, with complete Certificate of Product Registration (CPR) &
Material Safety Data Sheet (MSDS).

b. Conduct perimeter residual surface spraying on base walls of the building, perimeter fence
and garden / landscape to control insect pest harboring in the area.
c. Installation of New Rat Bait Stations with corresponding identification card number along the
perimeter wall and perimeter fence of the building at 15 to 20 feet apart. The rodenticide to be
used shall be Klerat wax block, Racumin paste and or Racumin Powder. Installation also of
Non-toxic Rat Glue traps (glue boards) inside sensitive areas of the new building using Rid Rat
Glue and Glue Boards. Monitoring of catches and replacement of baits shall be made at least every
other day or twice a week or when necessary.

d. Conduct thorough Thermal Fogging or Misting in the above mentioned new Phase II building and
areas indoor and outdoor including perimeter fence to control all flying and crawling pest. The
chemicals to be used shall be Temprid or DeltaThor and or Aqua Resigen. The application of
treatment shall be at least twice a month preferably non-working day (Sundays) or when necessary.

e. Termite Prevention and Control in some areas susceptible to termite infestation like STG,
ESP Electrical & Control Building, Switch Yard, Cooling Tower Electrical Room, SNCR, Bed and Fly
Ash Storage, Cooling Tower areas and other Control Rooms,. The Chemicals to be used shall be:
Premise TC, Agenda and or Maxx Thor.

f. Installation of Insect Light Traps in selected areas of the new Phase II building for the control
and monitoring of pest in the area.

g. Installation of Bird Control traps and repellent in selected areas to control, prevent and drive
away birds harboring the area.

Two (2) assigned Pest Control Technician will do the daily pest control activity and monitoring on the
above mentioned Phase II areas.

The Total Cost and Computation for the whole treatment to include labor, Materials, chemicals,
equipment’s and implements for the New Phase II Building and Areas for One (1) Year Contract
is at Php 384,000.00. (at Php 32,000.00/month)
