Dear Sir:

This is to formally inform you that our company is a duly licensed Pest Control Applicator: Commercial
Fumigator/ Exterminator and dealer of household and structural pest control with BFAD LTO No.: RDI-RXI-
HHS/W-446. And Exclusive Distributor and dealer of Bayer, Syngenta and 3M products.

For your general pest control maintenance requirement, specifically to control flies, mosquitoes, cockroaches,
ants, bugs, lizards, termites and other flying and crawling insect pest in the entire plant premises to include
rodent eradication and control. Below, we are pleased to offer you our services:

Scope of Works:

a. Residual Spraying to prevent and control insect pest such as flies, ants, cockroaches, mosquitoes, lizard
and other flying and crawling insects using environment friendly, biodegradable with low mammalian
toxicity chemicals of BAYER products such as; Temprid, SC, Premise TC, Agenda for termite
prevention and control, Maxforce Forte & Quantum Gel for cockroach and ant control, Racumin Paste
& Powder for Rodent Control. We are also going to use the product of Syngenta for alternative use like,
Klerat for rodent, Icon EC/SC for vermin control. We also have Ensystex Product as another alternative
like; Delta Thor Plus & Cyflux EW, Vectocide & Stargard for Vermin control, MaxxThor for termiticide
and insecticide use. These products are Approved and registered with Food & Drugs Administration
(FDA) and Fertilizer and Pesticide Authority (FPA) for Household and Industrial use, with complete
Certificate of Product Registration (CPR) & Material Safety Data Sheet (MSDS).

Using alternative product or pesticides for the purpose of avoiding pest resistance.
b. Conduct perimeter residual surface spraying on base walls of the building, perimeter fence and
garden / landscape to control insect pest harboring in the area. (please see attached pest control
program)

c. Installation of Rat Bait Stations with corresponding identification number along perimeter fence and
perimeter walls of the building at 15 to 20 feet apart. The rodenticide to be used shall be Klerat wax
block and or Racumin paste or Racumin Powder. Installation also of Non-toxic Rat traps (glue
boards) inside sensitive areas like; Offices, Control Rooms, Commissary, Locker , Comfort Rooms, other
related areas using Rid Rat Glue and Glue Boards. Monitoring of catches and replacement of baits shall
be made at least every other day or twice a week or when necessary. (please refer to attached pest control
program)

d. Conduct thorough Thermal Fogging or Misting in the above mentioned areas of the power plant indoor
and outdoor areas including perimeter fence to control all flying insects. The chemicals to be used shall be
Icon EC, DeltaThor, Outlook EC . The application of treatment shall be at least twice a month preferably
non-working day (Sundays) or when necessary. (please refer to attached pest control program)

e. Termite Prevention and Control in some areas susceptible to termite infestation like MCB, STG
Building, Admin Building, Offices, Control Rooms, Laboratory and undergrounds. The Chemicals to be
used shall be: Premise TC, Agenda and Maxx Thor.

f. Installation of Insect Light Traps in selected areas for the control and monitoring of pest in the area.

g. Installation of Bird Control in selected areas of the plant to control, prevent and drive away birds
harboring the area.

h. We would like to ask from your company a Pest Control Room within the compound for our materials,
equipment, chemicals and PPE’s.

There will be an assigned Two (2) Pest control technician to do the daily pest control service on the above
mentioned areas.

The Total Cost and computation for the whole treatment to include labor, Materials, chemicals, equipment’s
and implements for One (1) Year Contract is at Php 1,590,000.00.
