AFFIDAVIT OF GUARDIANSHIP

I, LORENA DUREMDES RICO, married, of legal age, Filipino, and resident

and with postal address at Brgy. Perez, Calauan, Laguna, after having duly sworn to in
accordance with law, do hereby depose and state:

1. That I am the auntie and the guardian of the DAEYEONG RICO KIM, who
is fifteen (15) years old, having been born on April 8, 2004;

2. That his parents WONKIL PARK KIM and RONALIE RICO KIM is working
abroad;

3. That while his parents are working in abroad, his custody is left to me;

4. That there is no other guardian for the said DAEYEONG RICO KIM;

5. That the said DAEYEONG RICO KIM has not been adopted by another
person;

6. That I have never been convicted of any crime which carries with it the
penalty of civil interdiction;

7. That I executed this Affidavit to attest to the truth of the foregoing and for
any legal purpose it may serve;
